The Transition of Care Across the Continuum: Optimizing Care for Older Adults

One in five hospitalized Medicare beneficiaries is readmitted within 30 days of discharge from an acute care setting at a cost of $2.6 billion annually; three quarters of these readmissions are potentially avoidable. HealthStream partnered with Duke Health to study ways to optimize the care of this important and growing population. The Duke Center for Aging and Human Development is one of five research centers established by the Surgeon General of the United States in 1955. Both the schools of nursing and medicine at Duke are part of this center and are continually ranked among the top ten programs in the country, with dedicated programs in gerontology.
